[LVT Intake Exam] Microchip Scan: negative, placed 985113001872621; Evidence of Cruelty: no, neglected; Observed Behavior: allowed all handling somewhat shut down; Sex: intact female; Estimated Age: appx 10y+; Subjective: found wandering by PO, no history or known owner; dog had “boot” matting over limbs and moderate matting over body, overgrown nails were interwoven with hair, groomed some on intake to free feet, soaked wounds in novalsan; Eyes: opacity (cataracts vs nuclear sclerosis) Ears: some yellow waxy d/c; Oral Exam: moderate staining, loose insertions; Heart: possible abnormal heart sounds (panting heavily throughout) Lungs: WNL; Abdomen: WNL; Musculoskeletal: WNL BCS 5/9; Mentation: BARH; Preliminary Assessment: dermatitis from poor haircoat, wounds associated with shaving/embedded nails; Plan: DVM intake, follow up grooming, picture and topical preventatives when bathed
