No History : Owner evicted, on hold Subjective: QAR-H, nervous Observed Behavior – nervous during exam, limited facial exam, teeth barred when attempt oral exam Objective T = P = 140 R = eup BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: severel calculus on rostral teeth – limited view of caudal teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated but very tense U/G: no testicles palpated MSI: Ambulatory x 4, skin free of parasites, no masses noted, sparse coat, very dirty, some matts, nails very overgrown; embedded nails on front digit 1’s (on right, severely embedded such that nail has curved under and completely punctured the dorsal surface of paw, leaving a full thickness tissue defect when nail clipped), infected; marked generalized muscle atrophy CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 8 year old CM Terrier X 1. Poor BCS r/o systemic illness v inadequate nutrition v other 2. Severe calculus 3. Unkempt, dirty 4. Embedded nails – severe on digit 1 RF Plan -clip nails todays; clip fur around RF and clean wound associated with embedded nail -start on clavamox x 5 days for embedded nail injury -tramadol 12.5 mg PO BID (add on rimadyl if bw okay) -E-collar -Rabies, Bordatella, DHPP -MC -HWT -actyvil, pyrantel -recommend cbc/chemistry/T4 Prognosis: Unknown pending reason for emaciation SURGERY: Okay for surgery when off hold
