Medical Summary SCAN NEGATIVE INTACT FEMALE APPROX. 1-2 YRS BAR, COOPERATIVE FOR EXAM CLEAN DENTITION CLEAN AU CLEAR OU BCS = 3/9 – UNDERWEIGHT H/L = NMA, NSR, PSS, CLEAR LUNG FIELDS EMBEDDED COLLAR – WOUND IS COVERED IN DEBRIS WITH METAL CHOKE CHAIN – UNSURE OF DEPTH – PATIENT REQUIRES SEDATION/ANESTHESIA FOR REMOVAL. LIKELY SUTURES NEEDED TO FACILITATE HEALING NO MGTS, ENLARGED VULVA BUT NO VAGINAL DISCHARGE A: EMBEDDED COLLAR PX: GOOD P: 0.8 ML RIMADYL SQ AND 2 ML PEN G SQ ADMINISTERED YESTERDAY 1. 6 ML RIMADYL SQ AND 2 ML PEN G SQ ADMINISTERED THIS AFTERNOON APPEARS TO BE EATING WELL WILL SWITCH TO ORAL MEDICATION WILL INFORM NEW HOPE TO SEE IF PATIENT CAN BE PLACED AS SOON AS POSSIBLE TO REMOVE COLLAR – WILL ATTEMPT TO ANESTHETIZE AND REMOVE COLLAR. ADDENDUM: SEDATED PATIENT WITH 0.45 ML TELAZOL AND MAINTAINED ON ISOFLURANE. CLIPPED AND CLEANED NECK CIRCUMFERENTIALLY CHOKE CHAIN COLLAR SEEMED TO AFFECT ONLY SEGMENTAL AREAS OF THE DORSAL NECK EMBEDDED WOUNDS EXTENDED APPROXIMATELY 0.5 CM DEEP AND MEASURED APPROX. 0.5-1 CM IN WIDTH APPEARED TO HAVE SOME CAPILLARY EXPOSURE AND WHAT MAY HAVE BEEN FIBROUS TISSUE AT THE MOST DEEP AREA. CLEANED ALL WOUNDS WITH STERILE SALINE AND FLUSHED WITH NOLVASAN USING HIGH PRESSURE TECHNIQUE FRESHENED EDGES TO OBTAIN NEW TISSUE AND CLOSED WOUNDS IN A CRUCIATE PATTERN/SIMPLE INTERRUPTED USING 2-0 MONOSORB SUTURE. PATIENT RECOVERED FROM ANESTHESIA UNEVENTFULLY STARTED BAYTRIL – 1.5 ML IM SID X 10 DAYS
