Medical Summary 11/05/16 10:12 scan negative approx 10yo MI Amer PBT S:surrendered by owner, very sweet, defecated during exam = live roundworms seen in feces O: AU: mild dry waxy discarge OU: bilateral inferior entropion, mucous discharge & conreal opacity Oral: moderate calculi Int: thickned/lichinefied skin over scrotum, hyperkeritosis of nasal planum, severely overgrown nails causing twisting of digits and hypertrophy of digital pads, few epidemral collarettes on ventral abdomen, embedded pinch collar causing multiple deep punctures with hemorrhage on right side of neck and ventral neck just left of midline Lnn: WNL CV: NMA, s&s pulses, pink moist mm Resp: lungs clear x4 Abd: difficult to palpate due to obesity, live roundworms in feces Repro: intact male MS: 4x ambulatory with difficulty, BCS 9+/9 Neuro: PLRs, menace & mentation WNL, CPs WNL, no ataxia A: morbidly obese deep puncture wounds caused by embedded pinch collar entropion gi parasites P: flurosccein stain OU = nonspecific uptake = no ulceration seen pictures taken for possible cruelty collar removed wounds cleaned nails clipped 125mg Rimadyl PO BID x5days 1250mg Cephalexin PO BID x 7 days 120# dose Panacur PO SID x 3 days px: good for wound, needs weight loss
