Medical Summary S: initial exam; she was found in a home when the owner was taken to a hospital; she was brought to South Shore Animal Hospital and found to have a tooth root abscess, mammary gland tumors, mpl’s, skin disease, and thoracic/thoracolumbar pain; she was given a convenia injection, methadone injection, and SQ fluids; transferred from the South Shore to here; stat chem performed: slight hyponatermia 136 (normal 138-160), all other values wnl, PCV/TS= 35/6.4 O: BAR mm’s pink, moist severe dental disease, severe halitosis, swelling ventral to right eye; painful; bleeding and discharge seen from right upper carnassial tooth OU- clear AU- mild brown waxy debris, alopecia at outer pinnae no murmur, regular rhyhthm lungs clear and eupneic abdomen- snp F, no spay scar seen; approx 1 cm firm mammary gland tumor palpated in caudal left chain, approx 0.5 cm firm mammary gland tumor palpated in the caudal right chain large amount of scale along dorsum BCS 3-4/9; ambulatory; no pain elicited on palpation of spine microchip scan positive: 4909244933 A: mammary gland tumors- R/O benign vs malignant severe dental disease, swelling and discharge at right side of face- R/O tooth root abscess vs neoplasia vs other dermatitis underweight P: continue tramadol prescribed by South Shore Animal Hospital recommend full bloodwork and dental feed soft food (e.g a/d), ate some food when hand fed
