Scanned neg on intake, MC placed at that time History : Stray, finder had for 2 weeks, had vomiting, bloody stool and intermittent appetite Subjective: Alert, allows all handling. Observed Behavior – Relaxed body, allows handling Objective: BAR-H, MMs pink and moist, BCS 6/9EENT: Immature cataracts OU. Severe lichenification and stenosis AU. No nasal or ocular discharge noted. Oral Exam: Severe tartar. PLN: No enlargements noted. H/L: Grade 1-2/6 systolic murmur PMI *RIGHT* base. Resp sinus arrhythmia. Lungs quiet, but pt coughs regularly. ABD: Distended, possible cranial organomegaly vs. overall severe distension. No specific masses palpated. U/G: Male intact; MSI: Ambulatory x 4, generalized truncal alopecia – long black fur on the dorsal trunk but nowhere else. There is some fine light brown fuzz on the lateral thorax but nowhere else. Many epidermal collarettes. No masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally. Assessment: 1. Alopecia, pyoderma, otitis externa. 2. Dental disease. 3. Abd distension 4. Heart murmur. Prognosis: Fair; Plan: 1. Do CBC/chemistry, UA if possible, abd rads, chest rads 2. Clean ears and give Osurnia, repeat in 1 week 3. Give Simplicef 50 mg PO SID x10 days – extend if improving; SURGERY: Permanent waiver due to age and underlying conditions.
RE EXAM: Vet Notes: 5:11 PM; 3-view chest rads:; Heart appears enlarged. Vertebral heart score 10.7. Lungs are unremarkable. Trachea is very very slightly narrower in the thoracic inlet than in the thorax, but not severely compressed anywhere.; 2-view abd rads: Right kidney – mineralization; Liver has a round mass sticking off into the abdomen; Prostate is visible – enlarged and in abdomen; Bladder is very full; Rectal exam: prostate enlarged, palpable; CBC: Elevated retic count despite normal Hct (183 K/ul), leukocytosis, neutrophilia, monocytosis; Chem: Elevated T. protein, globs and albumin. Alk phos elevated 1.5x normal. Other liver values are normal, BUN/creat are normal. Cholesterol was not tested.
