Objective T = 101.7F P = WNL R = WNL BCS 5/9 EENT: Nuclear sclerosis, ears clean, no nasal or ocular discharge noted Oral Exam: Severe dental disease, gingivitis, tartar buildup PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, honking coughing after being handled ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, grade 4/4 MPL at left stifle, grade 3/4 MPL right stifle, skin free of parasites, small raised irregular mass at left side of neck, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities. Assessment: Geriatric,
luxated patella, coughing – R/O collapsing trachea vs other, dental disease, mass – R/O sebaceous cyst vs other Prognosis: Fair Plan: Scheduled blood work and radiographs for tomorrow. Continue to monitor while at BACC. SURGERY: Permanent waiver due to age
