DVM Intake Exam : Estimated age:5 Microchip noted on Intake?no Microchip Number (If Applicable): History :stray Subjective: Observed Behavior -playful, jumpy, wags tail Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective : T = P =80 R =pant BCS 5/9 EENT: eyes surrounded by deep folds (entropion?)OD has corneal scar and very red sclera, OS seems wnl Oral Exam:mod staining, halitosis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G:intact male MSI: Ambulatory x 4, skin free of parasites, interdigital proliferative skin CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: possible entropion, pododermatitis with proliferative skin. corneal scar Prognosis:good Plan:tab ou short term–if eye redness continues should see ophthalmologist. no treatment for feet SURGERY: Okay for surgery
