DVM Intake Exam : Estimated age: ~10 years Microchip noted on Intake? negative History : stray Subjective: QAR, ~5% dehydrated Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures Evidence of Cruelty seen – yes Evidence of Trauma seen – no Objective : P = wnl R = eupneic BCS 2/9 EENT: Eyes clear with nuclear sclerosis ou, ears clean, no nasal discharge noted Oral Exam: adult dentition with moderate dental disease and multiple fractured teeth, no oral lesions noted
PLN: No enlargements noted H/L: NSR, Grade 2/6 HM PMI L apical, CRT < 2, Lungs clear, eupneic ABD: soft non painful, no masses palpated U/G: FI, no MGTs but developed mammae, no vulvar d/c but has edematous vulva MSI: Ambulatory x 4 but paraparetic, skin free of parasites, no masses noted, mild pododermatitis x4, emaciated CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, Severe cachexia, Emaciation, Nuclear sclerosis , Dental disease, Grade 2/6 HM Plan: Continue to monitor while at BACC, Rec geriatric workup when placed, LRS 20ml/kg SQ once-reassess hydration tomorrow, Keep in medical for daily monitoring, Rec cardiology consult Prognosis: Poor SURGERY: permanent waiver from surgery due to age/illness

11/04/17 Vet Notes : Recheck overall condition
S/O: QAR. ~5-8% dehydration. Allows all handling. BCS 2/9 EENT: Nuclear sclerosis, no ocular or nasal discharge, pale pink tacky mm, moderate dental staining/wear HL: Grade 2/6 heart murmur, lungs clear ABD: slightly distended, non tender INTEG: Generalized dull hair coat MS: Ambulatory x 4 but weak, stiff; significant muscle wasting especially around head (facial, temporal muscles) UG: Female A: Geriatric, weak, muscle wasting, heart murmur P: Rec LRS 350ml. Scheduled Blood work, recheck tomorrow, consider abdominal/+/- chest radiographs Appears to have underlying issue – poor prognosis but diagnostics pending
