Medical Summary Estimated age: 10 years Microchip noted on Intake? brought in as a stray but appears recently groomed BAR  docile, good for exam; okay to touch feet  T = not taken P = WNL R = WNL BCS 7/9 EENT: OU-severe mucoid discharge, corneas appear dull; moderate-severe chemosis; STT <5 mm/min OU AD-multiple ceruminous gland overgrowths at external ear canal with erythema and some ulceration; ear canal stenotic with mucoid discharge, erythema and inflammation AS-ear canal severely stenotic, inflamed, mucoid discharge hyperkeratosis of nasal planum Oral Exam: mm pk, moist; CRT <2 sec; severe periodontal dz with significant tartar and gingival recession, especially along incisors and canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses appreciated U/G: intact male MSI: large, ulcerated cutaneous mass on R side of face with secondary infection round, cystic like mass off of anus at 11 o’ clock position (1/2 inch in diameter) erythema along scrotum ambulatory x 4, decreased hip extension with mild discomfort bilat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: rectal mass; no rectal exam performed Assessment 1. Severe otitis externa AU with cerminous gland overgrowth 2. KCS with secondary conjunctivitis 3. Ulcerated cutaneous mass 4. Rectal mass 5. Periodontal disease Plan flush ears EOD, tresaderm for 10 days AU optimmune, erythromycin OU BID simplicef 150 mg PO SID rec’d derm consult for evaluation of severe otitis rec’d mass removal of cutaneous/rectal mass Prognosis: guarded to poor SURGERY: Okay for surgery
