DVM Intake Exam:Estimated age: ~3yrs Microchip noted on Intake? Yes  History : Stray Subjective: BARHObserved Behavior – Active, difficult to handle, unable to place soft muzzle due to snapping, bearing teeth, towel used for restraint. Objective: P = WNL R = WNL BCS 5/9
EENT: Eyes clear (difficult to visualize due to matting), ears clean, no nasal or ocular discharge noted. Oral Exam: Teeth clean, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated. U/G: Spayed MSI: Ambulatory x4, skin free of parasites, no masses noted, matted hair coat around face  CNS: Mentation appropriate – no signs of neurologic abnormalities  Assessment: Mild matting, most significant around face  Prognosis: Excellent. Plan: Continue to monitor while at BACC. Will need sedation to shave face
