Medical Summary 2/12/17 Sex: Male intact Scan: Negative Estimated age: Reported to be 8 years old by previous owner Fleas: No flea dirt. S: Nervous, skittish – tense, toweled for exam and allows handling. Whines when ignored. O: BAR-H, BCS 4.5/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. Moderate tartar and gingivitis. PLNs: Not enlarged. H/L: Grade 4/6 systolic heart murmur, NSR. Eupnic, quiet lung sounds. Abd: Tense, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male intact, testicles soft and symmetrical. A: 1. Heart murmur 2. Dental disease Short-term prognosis: Good-fair SURGERY: Permanent waiver due to heart murmur. P: 1. CBC/chemistry 2. Recommend chest rads and evaluation by cardiologist, then schedule dental cleaning if it is appropriate 1088

Basic information
Bill is a unaltered 8 year old male Chihuahua. Owner bought him from a pet store when he was a puppy. Owner is surrendering due to moving out of the state. There are no injuries or health problems owner is aware about. Bill has not seen a vet in 6 years.
Socialization
around strangers, bill is shy for a few minutes and then warms up. He has only spent time with a 2 year old and is fearful with the child by running into his crate and barking. He has never interacted with the child. He plays gentle with adults. He spent time with a French Bulldog and is respectful with the dog. They play gentle and exuberantly together. He has not been to a dog park so behavior with other dogs is unknown. Bill has not been around cats, behavior is unknown. There is no bite history with a person or animal.
Behavior
Owner stated that bill barks too much when visitors come into the home. He isn’t bothered with storms or loud noises. Owner has not tried touching his food bowl, taking a bone, or toy out of his mouth. He isn’t bothered when owner takes bill off the furniture. When holding or restraining Bill, he barks. He isn’t bothered with bathing or brushing of coat. It is unknown how his behavior is with trimming of nails. He barks at unfamiliar approaches to yard, house, or family member.
For the new family to know
Owner described him as shy, mellow, and independent. He has a medium activity level and owner loves how trained and obedient he is. When owner is home, Bill rarely seeks attention. He likes playing with balls and squeaky toys while playing fetch and tug. He was kept indoors and sometimes outdoors when it was nice. Bill eats 2 cups of purina wet and dry food twice a day. He loves bacon flavor treats. He is very house trained and rarely has accidents in home. He goes outside on grass and cement. He was crated when left alone in the house. He is well behaved alone in the yard. He does well for 9 hours in a crate. He knows sit, his name, and come inside. He pulls lightly on leash and when off leash he wanders but comes when called.
Behavior upon intake
Bill was showing signs of fearful body language by having his tail tucked in and crouching on the floor but allowed to be scanned collared and picked up.
