Medical Summary DVM Intake Exam Estimated age: Could not examine teeth – muzzled for exam. Microchip noted on Intake? yes History: Found stray, no medical history available. Subjective: BAR, euhydrated, BCS 4/9 Observed Behavior – sweet but nervous. Muzzled for exam. Pawed at muzzle but handled it well. Resents handling of the feet. Evidence of Cruelty seen – possible, multiple excoriations and wounds Evidence of Trauma seen – possible, multiple excoriations Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: could not complete due to muzzle in place PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated, very tense U/G: neutered male, no discharge noted MSI: Ambulatory x 4, well-muscled. Many excoriations and wounds; worst on the right lateral stifle (2 cm x 5 cm), left medial thigh (2 cm x 5 cm), multiple 0.5 cm wounds on the left thorax; multiple 0.5 cm wounds on the head and face. Severe pododermatitis on the hind feet with multiple excoriations, worst on the left lateral 2nd digit and right medial 2nd digit. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: neutered male, severe pododermatitis and multiple wounds with possible traumatic origin. Plan: Start cephalexin 500 mg PO BID x 10 days. Prognosis: Excellent with proper medical management SURGERY: Not needed
