DVM Intake Exam; Estimated age: 8yrs based on O hx; consistent with exam; may be closer to 10yrs. Microchip noted on Intake? scanned negative by LVT on intake. MC placed on intake. History : Surrendered by O. P reported to have asthma and a hx of resource guarding with multiple bite incidents. Subjective / Observed Behavior – BAR, allowed all handling. Objective; BCS 5/9; EENT: Eyes clear, ears contain mild brown otic debris; no nasal or ocular discharge noted; alopecia and hyperpigmentation along the dorsal pinnae; Oral Exam: dc 4/5; pd 4/5 ; multiple missing teeth; PLN: No enlargements noted; H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact male. testicles are smooth and syemmtrical; MSI: Ambulatory x 4, skin free of parasites, no masses noted, multiple areas of hyperkeratosis, hyperpigmentation and alopecia, especially along pressure points. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal; Assessment; hyperpigmentation and alopecia — r/o skin allergies, 2nd to chronic irritation; dental disease; aural exudate — resolved with ear cleaning; Prognosis: good; Plan: await behavioral assessment; ear cleaning performed at intake. SURGERY: Permanent waiver due to age
