DVM Intake Exam: Estimated age: 8-10yrS. Microchip noted on Intake? no Microchip Number (If Applicable): History: Stray Subjective: Dog is BARH Observed Behavior – immediately social, allows all handling. Objective: T = NA P = wnl R = wnl. BCS 7-8/9 EENT: Eyes – large amount mucoid discharge OU; STT – 5mm/min OU; Ears – hyperpigmentation with dark waxy debris AU, crust and/or mass distal pinna AS; audible. congestion but no nasal discharge noted. Oral Exam: Grade 1-2/4 dental dz. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, difficult to palpate due to obesity. U/G: male, no palpable testicles. MSI: Ambulatory x 4, skin free of parasites, has yeasty smell, seborrheic dermatitis. CNS: Mentation appropriate -no signs of neurologic abnormalities. Rectal: not performed. Assessment: Overweight, KCS, Chronic dermatitis Prognosis: Fair-Good Plan: Flushed OU and applied Optimmune – continue BID indefinitely. Recheck STT in 1 week. Cleaned AU and applied Claro. Recheck 1 week. Recommend medicated baths (with combination chlorhexidine/ketoconazole) shampoo 2-3 times weekly. Recommend rescue placement. Recommend bloodwork +/- derm consult on placement. SURGERY: Appears already neutered

Summary (1): Belvedere was surrender as a stray so his past behavior with other dogs is unknown.
3/17- 3/18: When off leash at the Care Centers, Belvedere greets a group of small male and female dogs with a soft posture. He mostly kept to himself while exploring the pens.
