SAFE 10/08/16

Manhattan Center

My name is BELLE. My Animal ID # is A1092247.
I am a female tan and gray shih tzu mix. The shelter thinks I am about 10 YEARS old.

I came in the shelter as a STRAY on 10/03/2016 from NY 10035, owner surrender reason stated was STRAY.

MOST RECENT MEDICAL INFORMATION AND WEIGHT
10/05/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 14.0 LBS.

10/05/16 17:44 S/O: OD cherry eye, irritated, surface ulcerated. corneas edematous. OS crusty ropey dc c/w KCS moderate dental disease multiple mammary gland tumors throughout both chains dirty ears heart and lungs wnl A: MGT benign vs malignant severe ocular disease cherry eye, infection probably KCS dental disease P: fair to guarded prognosis. mammary tumors 50/50 chance of malignancy, excisonal biopies needed eye disease is conrllable but will require surgical intervention and long tem meds in house tx with tab, optimmune , rimadyl, buprenorphine 10/4 BARH scan negative female geriatric heavy tartar blind, wound on rt eye, very cloudy both eyes, occular d/c slightly matted dirty coat mammary mass long nails trimmed NOSF

10/03/16 20:24 BEHAVIOR DURING INTAKE: Belle came into our care center as a stray. She was found roaming a park and unleashed. The finder had asked around in an attempt to find the owner but had been unsuccessful. Belle is an older dog and appears to have some clouding in her eyes. During her intake she allowed the counselor to approach her and pet her. She ws actively walkign around the room and bumped into the gate, which may be indicative of a problem with her sight. She allowed the counselor to collar her and photograph her with ease.

Exam Type BEHAVIOR
Though Belle has remained social and friendly while here at the center, due to her medical concerns she is not a candidate for a SAFER assessment at this time. Belle must be placed with a New Hope partner.

10/06/2016 GROUP BEHAVIOR EVALUATION – NH ONLY
Exam Type GROUP BEHAVIOR
When off leash with other dogs Belle explores the pen, briefly sniffing dogs that she passes by. She offers correction when solicited with play.
