I am a female gray and white shih tzu mix. The shelter thinks I am about 2 YEARS
I came in the shelter as a STRAY on 08/06/2016 from NY 10039, owner surrender reason stated was STRAY.

08/09/2016 Exam Type RE-EXAM – Medical Rating is 4 C – SEVERE CONDITIONS , Behavior Rating is NONE, Weight 10.6 LBS.
S/O: QAR, nervous, wide-eyed appearance. Thoracic auscultation wnl, abdominal palp wnl. Urine staining on feet and ventrum. Normal cranial nerves, normal anal tone. Possibly mild ataxia, walking crouched (nervous?), possibly circling to the left. No neck or back pain elicited on palpation. Possible CP deficits in hind limbs although not willing to bear weight. No nasal discharge/sneezing. A: Young FI Shih Tzu Neuro abnormalities (possible circling to the L, possible ataxia, quiet mentation) Skin disease History of CIRDC Moderate leukocytosis- mature neutrophilia, monocytosis, basophilia Thrombocytosis P: Continue tx plan (finish buprenex, does not appear to need furthe rpain meds) monitor for changes in neuro status Recommend NH placement 08/08/16 10:30 S/O: QAR, continues to be stiff and uncoordinated, eating well, seems nervous but non-painful when at rest A:1-2 yo FI Shih Tzu Skin disease – suspect scalding Neurologically inappropriate, suspect painful – r/o GME vs spinal disease vs trauma vs other infectious vs referred vs neoplasia vs other CIRDC P: continue current tx plan recommend NH placement prognosis: fair 08/07/16 12:45 S/O: BAR, eating well, continues to show neurologic signs (very stiff posture, bucking backwards and losing balance), no neck or spinal pain palpated, urine staining on fur, wounds and scarring on vetral abdomen, no coughing this morning, seems painful on palpation of caudal back/abdomen, no murmur or arrythmias ausculted today A:1-2 yo FI Shih Tzu Skin disease – suspect scalding Neurologically inappropriate, suspect painful – r/o GME vs spinal disease vs trauma vs other infectious vs referred vs neoplasia vs other CIRDC P: continue bup and baytril CBC/chem – leukocytosis, chemistry WNL, heartworm negative recommend NH placement prognosis: fair depending on underlying disease keep in medical to monitor

08/06/16 15:21 Bella was found as a stray and brought into the ACC. Dog was carried in bag and brought to the back like this. She would not come out of the bag on her own, but allowed counselors to pet her. She walked out of the bag when the bag was tipped over. She allowed for collaring and pictures. She stood still on the table and allowed counselor to pick her up and put her on floor. Once on the floor, she would not follow counselor and hid under chairs and table. When someone came to take her to get vaccinations, she would not walk with them. She had to be picked up and carried. Her eyes were very wide while she was carried.

Exam Type BEHAVIOR
Due to Bella’s medical condition she is not a good candidate for a behavior assessment at this time. She has been tolerant of handling and examination thus far, but we have no information on her behavior outside the care center. We recommend placement with a New Hope partner who can address her medical conditions and then evaluate her behavior in a stable home environment before placement into a permanent home.
