DVM Intake Exam; Estimated age: 8; Microchip noted on Intake? no; History : surrendered d/t barking/landlord issues; Subjective: bar, bcs 3/9, euhydrated; Observed Behavior – friendly, compliant; Objective T = P = 144 R = eup, pant; BCS 3/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: severe periodontal disease, incisions and premolars falling out; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: externally wnl; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal:Assessment; 8 yo FS poodle mix, severe dental disease, BCS 3/9, otherwise apparently healthy; Prognosis: good; Plan: weight gain, dental cleaning with extractions in the future
