Medical Behavior: Blue, Sex: Spayed, Age: 13.8 years, Weight: 12 lbs
[LVT Intake Exam] Microchip Scan: positive 454E006E56; Observed Behavior: timid but allowed all handling; Sex: spayed female; Estimated Age: reported 14y; Subjective: owner reports multiple recent vet visits for possible “dementia” ddx and unpredictable behavior, animal underwent surgery last year for a mass removal that caused hairloss (questioned if adrenal tumor but owner thinks it was an enlarged ovary), possible arthritis (untreated); animal has hairloss in hinds and tail with hyperpigmented skin, stiff gait and overweight; Eyes: clear; Ears: clean; Oral Exam: underbite with moderate staining (head shy); Heart: hard to auscultate through pant; Lungs: WNL; Abdomen: fatty but WNL; Musculoskeletal: BCS 6.5/9; Mentation: BARH; Preliminary Assessment: overweight senior dog with unknown possible neurologic issue; Plan: DVM intake, withhold RV due to DOH status, withhold HWT due to regular preventatives
