Medical Summary DVM Intake Exam Estimated age: Reported to be 9 years old, dentition and physical condition are consistent with this Microchip noted on Intake? No MC noted on intake History : Surrendered, no health problems reported, last seen at vet 1 month ago for vx Subjective: Alert and appropriate. Observed Behavior – Panting, stands calmly for exam, relaxed body Objective BCS 8/9, MMs pink and moist EENT: 7 mm round mass in OS lower eyelid near the lateral canthus. Mild pigmentary keratitis on the sclera adjacent to the mass. No conjunctival erythema or discharge. Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Tense, no masses palpated U/G: Male neutered MSI: Ambulatory x 4, left hind leg shakes when pt is standing still. Moderate crepitus and medial buttressing in left stifle, medial buttressing but no crepitus in the right stifle. Mildly decreased extension in both hips. Skin free of parasites, no masses noted, healthy hair coat. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed Assessment 1. Overweight/obese 2. Arthritis in both knees, worse on left side, also likely arthritis in hips 3. Dental disease 4. Eyelid mass Plan 1. Recommend weight loss – this will decrease the stress on the joints and decrease Beau’s pain, as well as improve overall body health 2. Recommend dental cleaning 3. Consider mass aspirate or excisional biopsy Prognosis: Good 1088
