Medical Summary Estimated age: 7 years Microchip noted on Intake? scan negative History : stray Subjective: Observed Behavior – friendly BCS 4-5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: moderate dental calc PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Grade 1-2/4 lameness with hindlimbs, right stifle thickened and crepitus felt, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment hindlimb lameness- R/O osteoarthritis vs ruptured cruciate vs hip dysplasia vs other moderate dental disease Plan start tramadol 100 mg PO BID x 7 days Prognosis: fair SURGERY: Okay for surgery
