Medical Summary 05/22/17 10:30 DVM Intake Exam Estimated age: 7-8 yo female chi/dach cross Microchip noted on Intake? negative History : stray brought in emaciated, cruelty investigation possible Subjective: Observed Behavior – calm, allows full exam, no aggression, eating very well , urinating WNL Evidence of Cruelty seen – possible, emaciated/cachexic  Objective T = P = 160 R = eupnic BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: significant dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female intact, slightly swollen vulva, developed nipples no mammary gl masses palpable, no spay scar or tatoo MSI: Ambulatory x 4, skin free of parasites, no masses noted, rough hair coat CNS: mentation appropriate – no signs of neurologic abnormalities  Assessment emaciated Plan feed small frequent meals CBC/Chem Prognosis: good if no underlying disease SURGERY: Temporary waiver due to emaciation
