Medical Summary DVM Intake Exam 8/30/17 Estimated age: ~8yrs, female Microchip noted on Intake? No History : Owner surrender due to allergies Subjective: Observed Behavior – BARH. Attention seeking, allows all handling Objective P = WNL R = WNL BCS 4/9 EENT: Very early nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Missing upper left incisors, tooth roots exposed, otherwise mild dental tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated; ~2cm non reducible umbilical hernia U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, generaluzed dry, thin, brittle hair coat with few areas of alopecia and pustules; Grade 3/4 bilateral MPL CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Dermatitis, dental disease, luxated patellas Plan: Gave convenia 0.23ml Sq. Rec dental with placement. Continue to monitor while at BACC Prognosis: Good SURGERY: Permanent waiver due to multiple health conditions
