Medical Summary Estimated age: 12-14 yo Microchip noted on Intake? brought in as a stray QAR  docile, easy to examine; geriatric Evidence of Cruelty seen – emaciated  T = not taken P = WNL R = WNL BCS 2/9 EENT: OU-corneal deposits in mid field, no stain uptake or other active process; ears clean, no nasal discharge noted Oral Exam: mm pale pk; severe periodontal disease; mild hypersalivation PLN: prominent submandibular LN H/L: grade VI L sided systolic heart murmur; eupnic with normal lung sounds ABD: Non painful, no masses palpated U/G: neutered male MSI: ambulatory x 4, severe osteoarthritis, particularly in stifles (worse on R side) interdigital dermatitis with purulent d/c on all paws thin hair coat on hind end hyperpigmentation on abdomen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1.
