Yes 474A614F0E; History : Owner surrender; Subjective: BARH; Observed Behavior – Calm for exam and all handling, attention seeking, allows all handling; Objective ; P = WNL; R = WNL; BCS 5/9; EENT: Early nuclear sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam: Severe dental tartar, gingivitis, missing few incisors; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Suspect spayed, possible spay scar just below umbulicus; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Dental disease; Prognosis: Good; Plan: Continue to monitor while at BACC. Rec dental cleaning +/- extractions with placement. Trimmed nails; SURGERY:  Permanent waiver due to age (suspect spayed)
