Medical Summary SCAN POSITIVE (SEE MEMO) NEUTERED MALE APPROX. 2 YRS. PER NYPD, PATIENT WAS DARTED AFTER BEING INVOLVED IN A FIGHT WITH ANOTHER DOG (DECEASED ON INTAKE) RECOVERING FROM SEDATION ON PRESENTATION LARGE AMOUNT OF THICK BRIGHT RED VOMIT/SALIVA IN KENNEL, AROUND MOUTH AND ON FRONT PAWS DART PROTRUDING FROM LEFT FLANK – NOT BLEEDING SEVERAL BITE WOUNDS PRESENT AROUND FACE AND ALONG FORELIMBS – DIFFICULT TO MAKE THEM ALL OUT DUE TO DRIED BLOOD COVERING SOME OF THE WOUNDS PATIENT TOO RECOVERED FROM ANESTHESIA TO REMOVE THE DART – NEEDED TO SEDATE ONCE AGAIN HR = 48 BPM RR = EUPNEIC MM = PINK, MOIST ADMINISTERED 3.5 ML FLUMAZENIL (0.01 MG/KG) IV ONCE – NO EFFECT ON HEART RATE PLACED PATIENT ON ISOFLURANE CLIPPED AND FLUSHED ALL DEEP PENETRATING WOUNDS, INCLUDING DART WOUND, WITH NOLVASAN AND LRS APPLIED 1 SKIN SUTURE IN THE LEFT FLANK, AND 2 SKIN SUTURES IN THE LEFT CHEEK/NECK AREA OVER EACH WOUND (ALL WOUNDS WERE <0.5 CM IN DIAMETER) WOUND (<0.5 CM IN DIAMETER) OVER LATERAL RIGHT FRONT LIMB HAD APPROX. 2 -3 CM OF POCKETING VENTRALLY – PLACED A PENROSE DRAIN WOUND ON MEDIAL RFL JUST PROXIMAL TO THE ELBOW (0.5 – 1 CM IN DIAMETER) HAD VERY DEEP POCKETING THAT WENT DEEP TO THE MUSCLE TISSUE – DID NOT SEEM APPROPRIATE FOR DRAIN PLACEMENT – FLSUHED AND LEFT OPEN FOR ACTIVE DRAINING. SQ EMPHYSEMA STARTING TO FORM IN THE AXILLARY REGION ALL OTHER WOUNDS ON FORELIMBS APPEARED SUPERFICIAL. WOUNDS ON THE MUZZLE WERE LEFT TO CLOSE BY SECOND INTENTION HEALING. GAVE 3.5 ML PEN G SQ GAVE 3 ML RIMDAYL (50 MG/ML) SQ GAVE 2.1 ML BUPRENEX SQ TAB APPLIED OU – MODERATELY HYPEREMIC CONJUNCTIVA AND MILD SCLERAL HEMORRHAGING VACCINATED (NOT RABIES) RECOVERED UNEVENTFULLY – WILL RECHECK LIMB OVER NEXT SEVERAL DAYS
