Yes History : Vxn and exam w/DVM 11/18/17 Subjective: BAR Hydr wnl  Observed Behavior – Very easy to examine including mouth until rectal temp, restraint for venipuncture. Objective: T = 101.4 P = 120 R = wnl BCS 5/9 EENT: AU severely malodorous otitis w/creamy beige white liquid and firm polyp lesions at helix, Eyes clear, no nasal or ocular discharge noted Oral Exam: severely receeded gums incisors, mod to severe dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI nsf MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric
Otitis r/o chronic  Dental dz  Prognosis: good to guarded otitis Plan: Clean ears w/eye wash Simplicef 100 mg 1/4 tab po sid x 2 weeks SURGERY: Permanent waiver due to age
