Medical Summary DVM Intake Exam Estimated age: 5-7 Microchip noted on Intake? n History : stray Subjective: severe periphral lymphadenopathy, SMLN, prescap, popliteal, inguinal and random LN along thorax. OS>OD infiltrative process. OS eye is opaque, OD aqueous flare/ hyphema present Observed Behavior – calm, allows exam Objective P = 120 R = wnl BCS 2/9 EENT: see above for eyes Oral Exam: moderate tartar PLN: severe enlargement of all LN H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: unable to tell if bilateral cryptorchid or merely enlarged LN –prob neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: probably lymphoma but systemwide infection is a (less likely) possibility Plan: palliative care can be attempted if NH placement found–remission can be hoped for on a temporary basis. systmeic anitbiotics will be started to try to r/o infection cbc: wbc 22.8 K monocytosis, increased NO, mild anemia chem essentially normla except low bg @ 29 Prognosis: long term poor SURGERY: Permanent waiver due to illness waive all intake shots etc for now
