Medical Summary DVM Intake Exam Estimated age: 9yrs Microchip noted on Intake? yes (985121002865553) History: O/S – owners stated that the past 2 months Ariel has been going through on and off bouts of not eating and then diarrhea. They have not taken her to the vet due to cost and had been treating her at home by syringe feeding her water and wet food. She has recently been favoring her right side and has been lethargic, as well as experiencing weight loss. Visible eye discharge. Subjective: Dog is QAR/depressed Observed Behavior – Minimal to no restraint needed for exam and treatments. Dog allows all handling. Objective T = 102.4F P = 150 R = 70 BCS 1-2/9 EENT: Eyes – mucoid to mucopurulent ocular discharge with large crusts at medial canthus OU, moderate chemosis; ears clean; no nasal discharge noted; mild dry cough on tracheal palpation Oral Exam: Grade 4/4 dental dz; mm-pink, moist PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: no palpable bladder; peri-vulvar area is extremely matted/crusted MSI: Ambulatory x 4, but very weak, skin free of parasites, no masses noted, coat unkempt – multiple mild-severe mats CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Emaciated, Lethargic Plan: Parvo Test – Neg. STT – OD 18mm/min, OS 10mm/min Prognosis: Fair-Poor SURGERY: Already spayed
