Medical Summary DVM Intake Exam Estimated age: 11 Microchip noted on Intake? neg History : Found wandering the streets – stray Subjective: Barh Observed Behavior – Sweet but a little scared. Difficulty seeing. Objective T = P = 120 R = eup BCS= 5/9 EENT: Eyes – marked nuclear sclerosis, decreased PLR +. Ears clean, no nasal discharge noted Oral Exam: Marked tartar, mod gingivitis. Marked halitosis. PLN: No enlargements noted H/L: NSA, 3/6 murmur, CRT < 2, Lungs clear bilat, eupnic ABD: Non painful, no masses palpated U/G: neutered male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Very long nails but not embedded or impeding gait. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment: 11 yo neutered male Corgi/ Terrier x 3/6 murmur. Marked ddz. Long tails Plan: Routine intake. Nail trim. Recom dental prophylaxis son but not urgent. Recom monitor and work-up for heart murmur first plus BW. Prognosis: Good to fair.
