Medical Summary S: initial exam; microchip scan: negative O: BAR mm’s pink, moist severe dental calc; no nasal discharge OU- lenticular sclerosis AU- mild brown waxy debris no murmur, regular rhythm lungs clear and eupneic, referred upper airway sounds abdomen- tense, mildly distended MI, left testicle enlarged compared to right testicle; rectal- formed stool, unable to palpate prostate alopecia and comodones at ventral abdomen; hypotrichosis at tail; approx 1 cm cutaneous mass with hardened discharge at left shoulder A: severe dental disease enlarged testicle- R/O neoplasia vs other alopecia and comodones at ventral abdomen- R/O Cushings vs other cutaneous mass- R/O cyst vs other mildly distended abdomen- R/O Cushings vs neoplasia vs ascites vs other P: CBC: thrombocytosis (PLT= 600 K,microL), increased reticulocytes, rest wnl Chemistry: mild hyperglobulinemia (4.7), severely increased Alkphos (1704 U/L)- R/O Cushings vs age-related changes vs chronic hepatitis vs neoplasia vs other, rest wnl including ALT, GGT, and total bilirubin recommend abdominal ultrasound, ACTH stim or LDDS test; recommend to consider neuter and dental after further diagnostics
