Medical Summary DVM Intake Exam Estimated age: 3-6 years based on size and overall appearance Microchip noted on Intake? Scanned pos on intake! History : DOH hold for biting a dog and person. Stray, no hx. Subjective: Growls in the kennel. On leash, pt is strong and pulls. Lifts front leg as sign of anxiety. Allowed muzzling and all exam with no sign of aggression. Objective BAR-H, MMs pink, BCS 6/9 EENT: Moderate brown waxy debris AU, minimal erythema. Eyes clear, no nasal discharge noted Oral Exam: Muzzled PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male neutered MSI: Large (4 cm dia) thickened callus on the lateral aspect of the left stifle. No crepitus, decreased range of motion or thickening in the hips or knees. Sits with left leg to the side. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Likely otitis externa 2. Callus on lateral left stifle – possibly due to the way pt sits which could be related to arthritic changes Plan: 1. Cleaned ears and instilled Osurnia AU today – repeat in 1 week 2. Monitor for lameness Prognosis: Good 1088
