Medical Summary Estimated age: 14 years old Microchip noted on Intake? yes, 0A106A1F25. Stray, BARH. Tries to bite with all handling including microchip scan. Makes gums bleed when trying to bite. Will become cyanotic with prolonged handling. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 140 bpm R = 40 bpm BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: very limited but gums bleeding from dog trying to bite during exam, most of teeth missing but there is one mandibular molar on the right visible. Gums cyanotic during handling. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FS, very small hooded vulva, was unable to shave to look for scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, mildly matted haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Severe dental disease Geriatric Plan: Needs behavioral evaluation Prognosis: Good
