Medical Summary Estimated age: 12 years Microchip noted on Intake? no History : stray Subjective: bar, hydrated Observed Behavior – timid, frightened, needs firm restraint for exam/treatment P = 120 R = 20 BCS: 6/9 EENT: clear OU ears: clear AU, no nasal discharge noted Oral Exam: mild staining/tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, no ectoparasites noted, no masses noted, healthy hair coat. Embedded nails, worst is dewclaw left front (digit 1). Digit is firm/inflammed, rule out neoplasia vs infex/inflamm CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: embedded nails. Possible mass digit #1 left front. Plan: trim nails scrub digit 1 left front recommend antibiotics and analgesics, consider surgical excision/biopsy if not improving. it would be difficult to soak/scrub digit due to tempermant at this time. Consider later if becomes less frightened. continue to monitor at BACC Prognosis: fair to good depending on response to treatment SURGERY:Permanent waiver due to age
