Subjective: Observed Behavior – Calm, quiet, allows all handling. Objective: QARH. mm=pink, moist, CRT<2 s. HR=100, RR=pant. BCS=9/9. ORAL: Mod. calculus, gingivitis, dental wear. EENT: Nuclear sclerosis OU. No oculonasal discharge, no sneezing. Ears clean AU. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias but difficult to hear heart sounds due to panting. No coughing. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, spay scar found. MS: Ambulatory x 4, but somewhat reluctant to walk and weak in rear limbs. Mod. stifle thickening bilaterally, left stifle crepitus. INTEG: Full, damp and dirty haircoat. Approx. 6 cm soft, mobile, SQ mass ventral thorax at the level of the xiphoid. 1 cm irregular skin mass caudal left thigh. CNS: Mentation appropriate. A: Approx. 10 yr old dog with obesity, dental disease, hind limb lameness/weakness, evidence of stifle DJD. P: Rimadyl 120 mg SQ administered. Continue Rimadyl 100 mg 1.25 tab PO q 24 hours x 7 days starting tomorrow. House in padded large kennel. Schedule CBC/CHEM/T4. Fair prognosis with appropriate management.
