I came in the shelter as a STRAY on 08/08/2017 from NY 10460, owner surrender reason stated was STRAY.

08/11/17 08:20 S/O: visual exam only d/t temperment, unable to medicate with eye meds d/t temperment BAR, amb x4 no CIRDC signs EENT: eyes open and comfortable, no d/c seen at this time M/S: amb x4, LH- mild swelling of distal limb with ulcerative wounds over lateral aspect- appears to have a granulation bed on wounds A: wound P: OD: r/o infection, other CWSC 8/10/17 Re-exam after shavedown; visual exam only due to temperament; unable to administer eye meds S/O QAR, resting in kennel good appetite no nasal discharge, sneezing or coughing noted OD-holding open; mucoid discharge present over cornea mid field, unable to clean or examine further without sedation; mild episcleral injection ulcerative wound along lateral aspect of distal LH with some flaking/crusting; good granulation bed present over some areas; no signficant discharge or swelling A wound ocular discharge P d/c TAB ointment due to inability to medicate Continue with Tramadol, Rimadyl as previously prescribed shavedown performed by Dr. 0577 but it appears that wound on LH may be secondary to strangulation from matted condition

08/08/17 17:37 Alixx is a white small dog who was found alone in the park. She allowed finder to walk her but when she tried to pick him up to place into car he snapped at her hand. Alixx came into the shelter and did not do much besides sniff around during intake. he was calm when being scanned but did not wanto walk on leash with counselor and needed to be carried which he was fine with. He allowed counselor to collar him with no problem but when counselor attempted to move or cut some of the matts from face for photograph he would snapp every time at the hand.

KNOWN HISTORY: None 8/8/17 Unaltered Male, Stray SAFER ASSESSMENT: 8/10/17 Look: 5. Dog snaps. Toy: 1. Minimal interest, dog sniffs toy. Summary: Alixx snapped during look when the assessor attempted to touch him, so the handling portion of the assessment was not conducted. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: Summary INTAKE BEHAVIOR Upon intake, Alixx was calm and allowed handling. MEDICAL BEHAVIOR: 8/8/17 During his initial medical exam, Alixx was tense, showing teeth, and snapping. ENERGY LEVEL: We have no history on Alixx so we cannot be certain of his behavior in a home environment. In the care center, he displays a low-medium level of activity. RECOMMENDATIONS: New Hope Only _X_No children (under 13): Due to how uncomfortable Alixx is currently with touch and novel stimuli, we feel that an adult-only home would be most beneficial at this time. _X_Place with a New Hope partner: Alixx has not acclimated well to the kennel environment and has allowed only minimal handling since intake. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement-based) and re-evaluate behavior in a stable home environment before placement into a permanent home. Potential challenges: _X_Fearful/potential for defensive aggression: Alixx has attempted to bite during her stay in our care centers when uncomfortable. It is important to always go slow and give Alixx the option to walk away from any social interaction. Alixx should never be forced to approach anything that he is uncomfortable with or to submit to petting or handling. It should always be Alixx’s choice to approach a new person or thing. Alixx would do best in an initially calm and quiet home environment and should be given time to acclimate to his new surroundings.

DVM Intake Exam Estimated age: mid-age adult, 3-5 yr Microchip noted on Intake? scan negative History : found stray, severely matted Subjective: QAR Observed Behavior – very aggressive, sits quietly then lunges and tries to bite leash/hand, anything that touches or come near him, had to sedate to handle/PE Evidence of Cruelty seen – yes, severe matting with wound at lateral side of left hind leg Evidence of Trauma seen – yes, wound at lateral side of left hind leg Objective BCS 5/9 EENT: OD small corneal ulcer; AU mod ceriminous (yellowish) waxy d/c; no nasal discharge noted Oral Exam: mild tartar, missing both max. canine at level of gumline PLN: No enlargements noted H/L: under sedation, nsf – NMA; Lungs clear ABD: soft abd, no masses palpated U/G: intact male, both testes descended MSI: Ambulatory x 4 but reluctant to walk with all the matting; severe generalized matted coat (photos taken) – heavay cast like matting over all four limbs and tail, large matts on both sides of body and heavy matting around face/ears – long toe nails – a large wound/sore covers the lateral aspect of the left thigh and there is also ~ 2×3 cm section of thick granulation tissue on the lateral side of the left leg below the stifle –> a large amount of maggots were present at these wound (photos were taken) CNS: mentation appropriate – very aggressive – no obvious signs of neurologic abnormalities Rectal: not performed Assessment – severe matting – left hind leg wounds – OD corneal ulcer Tx: sedated with Torb/DexDom 0.14/0.14 IM, intubated and maint on O2 +/- Iso (as needed) for complete exam, grooming and wound management (gave Antisedan 0.04 mL half way through tx) – complete body hair cut – wounds at left hind limb flushed and cleaned with dilute chlorhex solution and saline + SSD applied – AU ears cleaned – OD medicated with atb ointment – e-collar placed – gave Convenia 0.65 mL SQ + Rimadyl 0.3 mL SQ + Hydro 0.15 mL IM Plan: recheck comfort, taking oral meds, keep wound clean and evaluate ability to treat eye ulcer Prognosis: good SURGERY: Okay for surgery
