Medical Summary Estimated age: 6yr Microchip noted on Intake none History : found on highway Subjective: stray Observed Behavior – timid , trembling Evidence of Trauma seen – lameness Objective BARH P = WNL R = WNL BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild dental tartar periodontal erythema PLN: No enlargements noted H/L: NSR, III/VI sternal HM , CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: BCS 5/9 Ambulatory x 4, however greatly reduced ROM bilat pelvic limbs. MPL grade 4 bilat, no bruising, or resentment from pet on extension of hips and stifles,no heat or swelling crouched hyperflexed gait and posture pelvic limbs, L worse than R, grade 4/4 patellar luxation, nonpainful on palpation, no bruising seen bilat stifle valgus deformity, MPLs, possbly complicated by ACL rupture and “permanent” cranial drawer left stifle live fleas and dead fleas seen no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment r/o congenital varus deformity, MPLs, possbly complicated by chronic ACL rupture left no treatment needed while at ACC HM r/o valvular Plan XR- sedated with 0.1ml butorphanol IV right lateral and DV views of pelvis stifles: bilat stifle valgus deformity, MPLs, possibly complicated by ACL rupture and “permanent” cranial drawer left stifle pelvis mild incongruity femoral heads, no fractures seen consider ortho consult at rescue/adoption, may be helped by surgical treatment cardio consult Prognosis: fair SURGERY: Permanent waiver due to heart murmur
