Vet Notes: 11:30 AM: DVM Intake Exam: Estimated age: 10. Microchip noted on Intake? No. Microchip Number (If Applicable) History : Stray Subjective: BAR, S++. Observed Behavior – Very nervous. Objective: T = 100.9P = 104 R = BCS 4/9. EENT: NS OU, mild epiphora OU, ears clean, mild nasalserous dc. Oral Exam: does not tolerate exam- severe dental tartar, recessed gingiva, halitosis, did not see a communication to external wound on the upper lip. PLN: No enlargements noted. H/L: no obv murmur, NSR, NMA, CRT < 2, Lungs clear, eupniec did not appreciate wheezes or crackles ABD: tense but Non painful, no masses palpated. U/G: enlarged teats and vulva, no vaginal discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, appropriate hair coat 1cm open wound ventral to right eye- with scab on top, bloody purulent discharge underneath, did not appreciate commnication to oral cavity but difficult to assess. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: normal externally. Assessment: R facial wound r/o infected wound vs other. Severe dental disease. CIRDC Intact female Geriatric Prognosis: Fair Plan: Clavamox 62.5 mg PO BID x 10 days SURGERY: Okay for surgery Temporary waiver due to Permanent waiver due to age
