11/11/16 14:57 Basic Info: Bella A1096490 is a 3yr old female black Chihuahua who was given to the owner by a friend 2 1/2yrs ago and is being surrendered along with her puppies Ban A1096494 & Talie A1096493, Chiquito A1096492 & Cosita A1096491 due to owner health, etc. Bella was last seen by a veterinarian a couple of yrs ago and currently has no known health issues/injuries. Socialization: Bella is friendly/outgoing with strangers. She’s lived with children ages 3yrs, 7yrs & 10yrs and was relaxed, gentle, playful and exuberant with them, the 4 Chihuahuas and the adults in the home. Bella hasn’t lived with cats but ignores the ones she sees on the street. She’s never bitten/scratched any person or animal. Behavior: During storms/fireworks Bella will bark also when someone knocks on the apt door. Bella isn’t bothered by the following: having her food/food bowl touched/moved, being brushed or if someone unfamiliar approaches the home/family member. She tries to jump out of the tub during bath time and also while having her nails trimmed. Bella hasn’t been restrained or disturbed while sleeping/resting. For a New Family to Know: Bella is described as friendly, affectionate, mellow with a medium-high activity level. Her favorite activity is to play chase/tag and be pet. She enjoys squeaky & stuffed toys. Bella’s a mostly indoor dog who’s walked 2x a day where she pulls on the leash and eats dry Purina dog chow 2x a day. She sleeps on a dog bed, is housebroken and well behaved when left home alone. Intake Behavior: Bella allowed all handling. She allowed to be scanned (-), collared and taken pictures of. She was picked up and placed into the kennel with ease.

Other notes: SAFER ASSESSMENT: 11/11/16 Summary: Upon entering the assessment room, Bella appeared timid though allowed petting and sought affection from assessor/handler. Upon collaring, Bella snarled and snapped towards the handler multiple times, for this reason, Bella is not an appropriate candidate to pursue the assessment for. Bella must be placed with a New Hope Rescue. PLAYGROUP: When off leash with other dogs Bella is tolerant of greeting but will spend her time exploring the yard. MEDICAL BEHAVIOR: 11/10/16 During her initial medical examination, Bella tolerated all handling without issue.
