11/08/16 16:21 BASIC INFO Yuki A1096284 is a 1 year old chihuahua mix. She was a gift to the owner 4 months ago from a friend. The main reason for surrender is that Yuki needs behavior modification because she has been urinating on beds for about a month. Owner stated also that for the last week, Yuki has only been picking at her food, not eating properly and throwing up. Yuki allows you to take away her food bowl, water bowl or toy away. Yuki listens to the command sit. when a stranger approaches the owner, she barks. For exercise she is walked on the leash. She is wee-wee pad trained and potty trained but has accidents. Yuki is fed Beneful dry dog food. she is best described as friendly, affectionate and playful. BEHAVIOR DURING INTAKE Yuki allowed all handling, collaring and photos.

11/10/2016 BEHAVIOR EVALUATION – EXPERIENCE
Exam Type BEHAVIOR
KNOWN HISTORY: Unaltered, owner surrender (with previous owner for 4 months) Previously lived with: Not reported Behavior toward children: Unknown Behavior toward dogs: Unknown Behavior toward cats: Unknown FG/RG: None reported Bite history: Not reported Housetrained: Has accidents, and recently (last month) has been urinated on the bed Energy level/descriptors: Yuki is described as friendly, affectionate, and playful Other notes: SAFER ASSESSMENT: 11/10/16 Look: 2. Dog’s eyes are averted. His body posture is stiff and fearful, his tail is low and not moving. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, his tail is between his legs, body stiff, mouth closed, lip long, ears likely back, may lip lick. Tag: 2. Playful, dog spins towards hand. Squeeze 1: 1. Dog does not respond at all for three seconds. Squeeze 2: 3. Dog yelps. Toy: 1. Minimal interest, dog sniffs toy. Summary: Yuki was a bit fearful and sensitive to handling during his assessment. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: Summary MEDICAL BEHAVIOR: 11/9/2016 Yuki was tense and nervous during her initial medical exam. ENERGY LEVEL: RECOMMENDATIONS: Experience (suitable for an adopter with some previous dog experience) _X_No young children (under 5): Due to Yuki’s timid and fearful behavior, we feel she may be intimidated by young children. Older, dog savvy children can be considered. Potential challenges: _X_House soiling: Yuki is reported to be urinating on the bed for the last month. We are unclear on what may have triggered this behavior change, and it does not sound as though any modification was attempted. We advise that Yuki will likely need guidance on house training. She will need consistent and frequent walks, rewards for eliminating outside, and careful watching when she is inside. She should never be punished for accidents, as that will set the house training process back.
11/11/2016 GROUP BEHAVIOR EVALUATION – EXPERIENCE
Exam Type GROUP BEHAVIOR
11/11: When off leash with other dogs Yuki will occasionally run through the pen but is overwhelmed when play is reciprocated. She will then calmly explore the yard. 11/12: Yuki will run through the pen by herself but does not engage in play. She seeks attention from handlers otherwise.
