DVM Intake Exam; Estimated age:8; Microchip noted on Intake? n; History : stray; Subjective: Observed Behavior -scared, very still. allows exam; Objective ; T =; P =80; R =wnl; BCS 8/9; EENT: sclerosis ou, cru, no nasal or ocular discharge noted; Oral Exam: mod staining; PLN: No enlargements noted; H/L: NSR, hm 2/6, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: neutered; MSI: Ambulatory x 4, skin free of parasites, large wart like mass on lhl oozing blood, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: otitis, obese, heart murmur, mass; Prognosis: good; Plan:monitor heart murmur. recc bloodwork in future including t4, weight loss; mometamax sid x 10d; mass is likely benign, but should be removed
