Medical Summary DVM Intake Exam Intact female Estimated age: 5 years Microchip noted on Intake? Negative History : Owner surrender. DOH. The patient bite a person. Subjective: Apparently healthy intact male dog. Observed Behavior – Fractious Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective T = Not taken P = 100 bpm R = Not taken BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Not examined. Muzzled PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Not examined Assessment Apparently healthy dog Behavior. Plan Vaccine Microchip Pyrantel in food Flea treatment DOH hold for 10 days Prognosis: Good Temporary waiver due to DOH for 10 days.
