Medical Summary DVM Intake Exam **limited exam – very aggressiove behavior – limited PE with muzzle and heavy restraint** Estimated age: mature adult, 4y Microchip noted on Intake? scan negative History : owner surrender Subjective: BARH Observed Behavior – nervous/aggressive behav – unable to handle for PE without muzzle/heavy restraint Objective P = wnl, excited R = wnl, panting BCS 5/9 EENT: Eyes clear, no nasal discharge noted Oral Exam: not performed PLN: not examined H/L: limited exam, nma, Lungs clear ABD: not palp. U/G: intact male MSI: Ambulatory x 4, skin free of parasites, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: very aggressive behav/limited PE – apparently healthy Plan: neuter – rec’ full PE under sedation – needs HWT when sedated Prognosis: good SURGERY: Okay for surgery
