Medical Summary DVM Intake Exam Estimated age: 2-3 Microchip noted on Intake? n History : in house with two other dogs. Subjective: All three dogs have evidence of bite wounds. this dogs wounds are very minor, mostly old scars on neck area Observed Behavior – wags tail, allows full exam Objective P = 100 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean teeth PLN: No enlargements noted H/L: NSR, NMA, , Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: no SS seen MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment; healthy Plan: OHE after doh hold Prognosis: excellent SURGERY: Okay for surgery
