DVM Intake Exam: Estimated age: 2 yo. Microchip noted on Intake? yes  History : Stray
Subjective: BARH, super sweet and friendly. Well behaved, no signs of aggression Observed Behavior –  Objective:  T = P = wnl R = eup BCS 5/9  EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: neg oral, clean dentition, minimal staining PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated U/G: FS, no MGTs, no vulvar discharge MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: NE. Assessment:apparently healthy Prognosis: excellent Plan:  ctm at bacc until transfer/adoption

Medical Summary Estimated age: ~1-2 years Microchip noted on Intake? negative History : stray. Was reported to have possible bloody diarrhea on rounds board but was likely bloody vulvar d/c from heat. No c/ss/v/d noted Subjective: BARH Observed Behavior – Very sweet and loose for exam until vulvar exam and then whipped head around. Was muzzled for medical procedures  P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI, serosanguineous vulvar d/c, no MGTs, developed mammary tissue MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy In heat Plan: Continue to monitor while at BACC Monitor heat-rec spay Prognosis: Excellent SURGERY: ok to schedule for surgery
