DVM Intake Exam; Estimated age: ~10yrs; Microchip noted on Intake? no; History : Stray; Subjective: Observed Behavior – BARH. Attention seeking, allows all handling; Objective P = WNL R = WNL BCS 5/9; EENT: nuclear sclerosis, ears clean, no nasal discharge noted; Oral Exam: severe dental tartar/gingivitis; PLN: No enlargements noted; H/L: Grade 3/6 heart murmur, normal sinus arrhythmia, CRT < 2, Lungs clear, eupnic, wheezing, mild cough when walking, quiet at rest; ABD: Non painful, no masses palpated; U/G: spay scar palpated, should be shaved to confirm; MSI: Ambulatory x 4, slight out-turning of both hind limbs, grade 3/4 luxated patellas bilaterally, skin free of parasites, no masses noted, healthy hair coat, overgrown nails; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric, dental disease, low grade heart murmur, luxated patellas, R/O collapsing trachea vs other; Plan: Continue to monitor while at BACC. Rec cardiology work up and dental with placement. Radiographs scheduled for tomorrow; Prognosis: Good short term prognosis
