DVM Intake Exam; Estimated age: 8yrs based on O hx and PE ; Microchip noted on Intake? scanned negative on intake. History : surrendered by O. O will be bed ridden for next 4 mos after sx and unable to care for P. Subjective / Observed Behavior: BAR; growling throughout exam. Very tense and shaking throughout entire exam. Objective ; BCS 5/9; EENT: Eyes clear; nuclear sclerosis OU; ears clean, no nasal or ocular discharge noted; Oral Exam: unable to perform due to P cooperation; P was muzzled ; PLN: No enlargements noted; H/L: NMA, Lungs clear, eupnic — difficult to auscult. growling throughout exam. ABD: Non painful, no masses palpated; U/G: intact male. smooth, symmetrical testicles. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal on visual exam. Assessment; intact male. apparently healthy ; dental disease ; nuclear sclerosis ; Prognosis: good. ; Plan: trazadone 50mg PO q12h x 4 days. behavioral assessment. SURGERY: Permanent waiver due to age.
