KNOWN HISTORY: None 7/11/17 Unaltered Male, Stray SAFER ASSESSMENT: 7/13/17 Toy: 1. No interest. Summary: Gilderoy appeared fearful in the assessment room, maintaining distance from the assessor. When the assessor reached towards him, he growled and bared teeth, so the handling portion of the assessment was not conducted. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: 7/13: When introduced off leash to dogs in the care center, Gilderoy approaches and greets politely. INTAKE BEHAVIOR: MEDICAL BEHAVIOR: 7/11/17 During his initial medical exam, Gilderoy growled and snapped. ENERGY LEVEL: We have no history on Gilderoy so we cannot be certain of his behavior in a home environment. In the care center, he appears to have a medium-low level of activity. RECOMMENDATIONS: New Hope Only _X_No children (under 13): Due to how uncomfortable Gilderoy is currently with touch and novel stimuli, we feel that an adult-only home would be most beneficial at this time. _X_Place with a New Hope partner: Gilderoy has not acclimated well to the kennel environment and has allowed only minimal handling since intake. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement-based) and re-evaluate behavior in a stable home environment before placement into a permanent home. Potential challenges: _X_Fearful/potential for defensive aggression: Gilderoy gives clear warnings when he is uncomfortable and does seem to choose to avoid or retreat when given the opportunity, but if prevented from moving away there is a potential to escalate to higher-level warning behaviors and possible fear-based aggression. It is important to move slowly with Gilderoy, to build positive associations (treats/toys/praise), and to allow Gilderoy to initiate interactions with new people. He should never be forced to greet or to interact if he is not comfortable and soliciting attention. IN-SHELTER OBSERVATIONS: Gilderoy has been observed to seek attention and offer affection to select handlers in the care center.

DVM Intake Exam Estimated age: 4-8 years based on white fur on muzzle Microchip noted on Intake? Scanned neg on intake History : Stray Subjective: Growling in kennel, hiding under a his kuranda bed that’s covered with a giant quilt. Everything in kennel removed in order to leash dog and bring him to exam room. In exam room – whale eye, still and stiff body, trembling. Difficult to muzzle, but muzzled for exam. Pt flinched whenever body was touched. Objective BAR-H, BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted. White fur on muzzle. Oral Exam: Not performed H/L: NSR, NMA Lungs clear, eupnic ABD: Tense, difficult to palpate U/G: Male intact, testicles S/S MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Unable to examine due to tucked tail Assessment: Apparently healthy Plan: Neuter Prognosis: Excellent SURGERY: Okay for surgery 1088
