Medical Summary DVM Intake Exam Estimated age: Reported 1 year and 3 months – exam is consistent with this Microchip noted on Intake? Scanned neg on intake History : Owner surrender. No health hx reported. Subjective: Alert, sits well. Observed Behavior – Quiet, reserved, tense. Muzzled as a precaution. Flinched away from hand placed on her head. No signs of aggression. Objective BAR-H, MMs pink and moist, BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Clean teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Mild upper resp stertor. Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, developed vulva, soft MGs MSI: Mild dermatitis and erythema on ventral neck and chest – flat, round areas of hyperpigmentation and erythema, no real epidermal collarettes or papules. Skin of ventrum and face is very pink. Ambulatory x 4, skin free of parasites, no masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Dermatitis, pink skin – R/O allergies to food vs. environmental triggers vs. other Prognosis: Good with long-term care as needed Plan: 1. Benadryl 50 mg PO BID x21 days. If this is effective, okay to continue. 2. Consider limited ingredient diet trial SURGERY: Okay for surgery 1088

A volunteer writes: Chloe is a young beauty, quite unique looking with her green lagoon dreamy eyes and her Boxer like face. She was surrendered into our care due to housing issues but comes with lovely comments. She lived happily with children and a large as well as a small breed dog and even a cat(always a bonus). Her previous owner said that she was very playful in her home, learned tricks fast and was friendly around strangers. Chloe is timid here at the care center. The change of scenery and the disappearance of the significant people in her life took a toll on her. Although appearing friendly in her kennel, she is quite hand shy and moves slowly, sometimes walking backward when she feels scared. She can stroll pretty fast though and even does stairs although her hinds seem to bother her at times. She did sit for me at the end of our time together and laid down beside me. I could look at her for ever and ever. She is so gorgeous and if I could paint, she would be my first model of choice. Chloe needs a home and to feel home with a master or family to love. The happy gal will then come back to life like Snow White with the kiss of a prince. Come and meet our lovely princess here at the Manhattan Care Center.
